How to ask clients about AI notes.
This is the consent form we use with our own clients, plus a handout that explains AI notes in plain language. Feel free to use them, even if you never use TheraBee.
- Consent script (editable .docx)
- Client-facing one-pager (PDF)
- State-by-state recording rules (PDF)
We recommend written consent everywhere, whatever your state requires.
Some states require every party to agree to a recording. The state map at the bottom of this page shows each rule.

The consent script.
You can add this to your intake paperwork or read it with your client at your first session together. Swap in your own practice name and make it sound like you. We’d just suggest keeping the details about how recordings and notes are deleted, since that’s usually what clients want to know most.
An editable version is included in the free kit.
Your therapist uses TheraBee, a HIPAA-compliant tool that uses artificial intelligence (AI) to help with clinical documentation. With your permission, TheraBee records and transcribes your sessions and drafts a clinical note. Your therapist reviews and edits every note before it becomes part of your record. TheraBee does not provide therapy or make decisions about your care.
The audio recording is deleted once the note is written. A transcript, with your name and other identifying details removed, is kept with the note in your therapist’s TheraBee account until your therapist deletes them. Your therapist has a Business Associate Agreement with TheraBee that requires it to protect your information as HIPAA requires. Your information is encrypted, is never sold, and is not used to train AI models.
Using TheraBee is a standard part of how your therapist provides care. If you’re not comfortable with it, please talk with your therapist. They’re glad to answer your questions and, if this isn’t the right fit for you, help you find another provider.
By signing below, I agree to my therapist’s use of TheraBee as described above.

The law allows recording with one person’s consent. We still recommend written consent from every client.
